On August 19, 2010, a 24-year old worker died at the Yankee Candle Co. warehouse in Deerfield, Massachusetts. He was operating a forklift when he fell 30 feet to his death. Richard Tyson, of Springfield, Massachusetts was pronounced dead at Baystate Franklin Medical Center in Greenfield.
His family stated that he was a hard-working, certified forklift operator who always put safety first. Apparently, the forklift was struck by another vehicle, causing the forklift to tip over.
This matter is currently being investigated by the Occupational Safety and Health Administration (OSHA). Many workplace accidents are the result of management’s failure to maintain a safe job site or are the result of defective equipment. Management must ensure that proper safety procedures are in place and that proper safety equipment is available.
